Self Evaluation Form

Bill created this self-evaluation form years ago as a way to give out a daily work grade. When he first started teaching he gave “gut” grades but did not have any documented grades that would support the grades that were given. The enclosed document was created to help eliminate this issue.
Each Friday before the hour begins students sit down and evaluate their efforts for the week and write on the back side at least two sentences on what they accomplished for the week towards the number of points that they assigned themselves.
When Bill sits down to record their points he double checks his record sheet where he keeps track of absences, tardies, no workdays, safety rule violations, etc. Sometimes students question what Bill has done but after he proves why he did the change they understand.
